PRIVACY POLICY

Information obtained by Simply Holistic Therapies will be collected lawfully and in accordance with the General Data Protection Regulations (GDPR)

With changes implemented in May 2018 under the General Data Protection Regulations (GDPR) this gives individuals the right to be informed about the collection and use of their personal data, which includes the collection of health information. This will covered in further detail during your first consultation visit with Simply Holistic Therapies.

Consent

By filling in the enquiry form on this website, you are giving your consent to Simply Holistic Therapies to hold your personal information and to make contact.

By signing the initial consultation form during your first visit, you are giving your consent for Simply Holistic Therapies to hold your personal information. The information which you supply on the forms will be held together with any clinical records or purchasing information, as applicable. I will use your information to give you the best possible service and for insurance purposes.

As a result of the Coronavirus (Covid-19) pandemic there will be an additional declaration form to complete confirming you have no current symptoms.

There will also be signed declaration from your therapist confirming they are fit and well.

 

Email marketing

With your permission, I will send you emails about my practice and other updates. You will need to positively opt in on your initial consultation forms to receive these updates. Storing your information

Your information is stored in my secure appointment management system and in a locked filing cabinet.  I do not store personal data on any portable devices such as a memory stick. All documents and applications that contain personal data are password protected.

Retaining your information 

You have the right to ask me to anonymise your records at any time if you wish to terminate our working relationship. I am required by law to retain adult records for eight years following our last date of contact and will destroy them with a shredder after that date. Children’s records are retained until their 25th or 26th birthday, depending on the exact age of the child when their treatment ended.

Accessing your information

Your records are accessed by myself for clinical purposes. Information provided online may also be accessed by my confidential website designer, who assists me with the upkeep of my website. You have the right to request a copy of your records at any time. These will be sent by email, in a Microsoft Word document attachment.

Sharing your information

I will never share your information without your permission – it will be kept strictly confidential unless there are legal grounds for me to disclose something which you have told me to a third party. I shall always inform you if I feel that disclosure is necessary and seek your permission and co-operation first.

With your agreed and written consent there may be instances where your medical information may be shared by the various healthcare providers involved in your on-going care and treatment.

Due to Coronavirus (Covid-19), should you contract the virus we are obliged to inform NHS Track & Trace.

Complaints

If, for any reason, you are unhappy with how I am handling your data, please raise your concerns with me first, so that we can seek a resolution. If following this you are still not satisfied, then you have the right to complain to the Information Commissioner’s Office (ICO).

I may amend this privacy policy from time to time when required by legislation to do so. You should check back periodically to check for any changes. This privacy policy is effective from the introduction of GDPR on 25th May 2018.
